CCDPH COVID-19 Community Vaccination Program Portal is Live


At noon on Monday, January 25, the Cook County Department of Public Health launched their COVID-19 Community Vaccination Program Portal. Through this site, suburban Cook County residents can sign up to connect with available vaccine sites and appointments. Vaccine Portal:

Visit https://vaccine.cookcountyil.gov/sign-up to sign up to receive information on which vaccination phase you are eligible for, updates on when administration expands to your phase, and, when it is your turn, information to schedule an appointment in your area.


Through this portal, you can register to receive alerts by email or phone with updates when there are eligible, open appointments in your area, based on vaccine availability. You will be able to schedule an appointment at a Cook County Health location directly through the website or be directed to partner locations, like Jewel-Osco and Walgreens, in your area. The portal also connects with other Cook County public health authorities, like Evanston and Chicago, to ensure that you are directed to the appropriate partners.


Vaccine Portal Hotline:

For individuals who are not as comfortable using a computer, a vaccine sign-up hotline has been created as well. Interested residents can call (833) 308-1988 to be connected with someone who can assist with the sign-up process.


This new portal and hotline are a massive step in the direction of returning to normalcy. While this is an exciting and important new development, it is imperative to remember that, at the moment, the desire for vaccines far outpaces the number of vaccines that are actually available. Please be patient. If, at first, you are unable to find a vaccine in your area, check back often as the site will update as new vaccine locations come online and as Illinois and Cook County receive additional doses of vaccine.
